Phimosis Tight Foreskin Treatment and Circumcision in London

As a consultant urological surgeon working in private practice, I regularly see patients with phimosis, a condition where the foreskin cannot be retracted comfortably over the head of the penis. This can affect children and adults and often requires assessment and treatment by an experienced urologist specialising in phimosis treatments.

Our London based clinic offers expert diagnosis, medical management, and phimosis surgery, including circumcision, frenuloplasty, preputioplasty and redo or revision circumcision when appropriate.

At my private urology clinic in London, we provide expert assessment, accurate urology diagnostics, and tailored treatments for phimosis to restore normal urinary function.

What is Phimosis?

Phimosis is the inability to retract the foreskin fully. It may occur naturally in young boys (childhood phimosis) but can also develop in adults (adult phimosis) due to scarring, inflammation, or infection.

Related conditions include:

  • Paraphimosis – when the foreskin is retracted and stuck behind the head of the penis, requiring urgent paraphimosis reduction.
  • Balanitis – inflammation of the foreskin and glans, sometimes requiring topical treatments.

Causes of Phimosis

The causes of phimosis vary and include:

  • Repeated infections such as foreskin infection or urinary tract infections
  • Chronic inflammation (balanitis, lichen sclerosus (BXO))
  • Foreskin scarring or tearing
  • Foreskin tightening in adults after injury or poor hygiene
  • Congenital tightness in boys with phimosis

Signs and Symptoms of Phimosis

Typical symptoms of phimosis include:

  • Pain or discomfort with foreskin retraction
  • Foreskin stuck to the head of the penis
  • Cracks or bleeding (foreskin cracking, foreskin tearing)
  • Recurrent foreskin infections
  • Difficulty cleaning under the foreskin
  • Foreskin tight when erect, painful erections, or sexual difficulty

Treatments of Phimosis

There are several treatment options for phimosis, depending on severity:

Non-Surgical Treatments

  • Phimosis treatment without surgery: steroid creams, gentle stretching exercises, and good hygiene.

Surgical Treatments

  • Circumcision is the most definitive treatment, involving the surgical removal of the foreskin.
  • Partial circumcision (removes only part of the foreskin).
  • Circumcision revision for patients with complications from previous surgery.
  • Foreskin sparing surgery such as frenuloplasty or preputioplasty

FAQS

What are urethral strictures?

A urethral stricture is a narrowing of the urethra, usually caused by scarring or inflammation. This narrowing restricts the flow of urine and may cause urinary problems such as a weak stream, difficulty passing urine, or repeated infections.

A stricture of the urethra refers to the same condition – a narrowing or blockage of the urethra that makes it harder to pass urine. It can occur in men or women and may require treatments such as dilation, internal urethrotomy, Optilume™ or urethroplasty open surgery.

Optilume™ is an innovative new treatment for urethral stricture disease. It uses a special balloon to gently widen the narrowed urethra. The balloon is coated with a medication that reduces scarring and helps prevent recurrence, offering a minimally invasive alternative to traditional surgery.
